Community Support   l   PREP   l   Jail Diversion

Community Support Services

Birmingham Group Health Services, Inc.'s Community Support Services offer the highest quality, individualized care whenever, and wherever, it is needed.  Participants in Community Support Services will have access to services and supports that address functional impairments affecting abilities to live independently in their chosen community. The services focus on skill acquisition and mastery, environmental adaptations to support recovery and accessing resources.  Services are offered in locations that meet the needs of participants. Such locations may include individuals’ homes, hospitals, shelters, correctional facilities and other community locations.   In addition, specialized services are available to individuals affected with co-occurring disorders and individuals involved in the criminal justice system.

The Community Support Services program receives funding from the Department of Mental Health and Addiction Services and offers a wide range of services to those individuals affected by mental health and substance abuse issues, or both (co-occurring disorders).  These services include, but are not limited to, skill building, assistance with benefits, housing, advocacy and support, and linkage to other services providers that can appropriately address each individual's needs.  In addition, case managers work collaboratively with other Birmingham Group Health Services' departments such as Rehabilitation, Clinical Services, and The Umbrella domestic violence program, to improve each individual's quality of life and ensure access to a full range of services to aid in his/her recovery.


  PREP Community Support Services

Birmingham Group Health Services,  Inc.'s Post Release Entry Program (PREP) provides services within the Support Service, Behavioral Health and Employment Networks for offenders in the New Haven Parole District. This “one-stop shopping” of comprehensive re-entry services include: substance abuse, mental health, case management, employment and skills development, financial management and transportation. The services offered are listed below.

Support Services:

  • Pre-release coordination (In-house) – Collaboration with parole, correctional facilities and CTDOC residential networks is provided to ensure that an appointment within 3 business days with the case manager is secured and that the LSI-R and the Re-Entry Workbook can be carefully reviewed.

  • ID Services (referred out) – Assists with transportation/completion of paperwork to obtain a license, ID or a birth certificate.

  • Food Vouchers (In-house + referred out) - Assists offender by providing BGHS donated food or applying for food stamps and/or accessing local food banks.

  • Housing Assistance (Referred out) –Emergency housing via homeless shelters (e.g. Columbus House); transitional housing via shelters (e.g. Areas Congregations Together) and direct services for fuel assistance, security deposit and eviction prevention (via TEAM, INC) is facilitated.

  • Benefits Assistance (In-house) – A BGHS Benefits Counselor assists with obtaining all medical and cash benefits.

  • Education (Referred out) – Educational needs are assessed via a Functional Literacy Test and a literacy volunteer is secured if necessary. Referrals for a GED are made to area adult education programs. Education beyond high school (e.g. community technical colleges) is also facilitated.

  • Transportation (In house + Referred out) – Provided when alternative transportation via Valley Transit District, CT Transit or a local cab company can not be secured. Bus tokens will also be available when appropriate.

  • Financial Management (In House) – Skill building regarding budgeting is offered as well as Representative Payee services.

  • Life Skills Development (In-House + Referred Out) – Social and life skills training is conducted on site (e.g. Life After Incarceration, budgeting, etc.) while AA, NA or parenting classes would be referred out to another agency

·       Behavioral Health:

  • Outpatient Substance Abuse Services (in-House)-An IDDT (Integrated Dual Diagnosis Treatment) mode of care is emphasized using a combination of MI, and CBT treatment.

  • Dual-Diagnosis Intensive Outpatient Program (In –House)- This 4-12 week program (3 hours a day for 4 days per week) combines step-wise groups with 12 step education, life management skills and family psycho-education groups.

  • Outpatient Anger Management Services (In-House) – A weekly12 session group designed for co-occurring anger and substance abuse problems or those with anger only problems. Teaches offenders to recognize triggers and problem solve to develop control plans and teaches cognitive restructuring techniques.

  • Urine Testing (In-house) - Conducted via rapid read urine cups and chain of custody.

  • Referrals (In-House) - Completed to a variety of in-house services (case management, benefits, vocational, etc.) as well as “referring out” for home health services, partial hospital or inpatient setting.

Employment

The following services will be offered in both individual and group settings- employment assistance services, employment counseling to enhance the offender’s marketability, career counseling, job search techniques, vocational training assessments, continued education, employability skills training, resume assembly, interviewing skills, application preparation, transportation arrangement and job retention techniques in addition to the following:

  • Vocational training  (In-House) –  Staff work collaboratively with CTDOC Job Center, CT Works and the Bureau of Rehabilitation Services which is on-site at BGHS.

  • Job Development (In-House) - Via BGHS experienced Job Developers, the employer network listing from CTDOC and the BGHS Business Advisory Council, linkage to employers who hire offenders will be facilitated.

  • Linkage with CT DOC Job Center (In-House) - Via regular meetings and visits, referrals made prior to release from a correctional institution will receive agreed upon pre-release services.

  • Referrals (In-House) -Upon admission, referrals are made to training programs as well as any other identified service. Upon discharge referrals will be made in accordance with the decisions of the offender and the PREP team.

Eligibility Criteria:
Parolees must be 18 years or older and referred by their parole officers or from the CTDOC Residential Community Network in which they reside.
All referrals must be made via BGHS Clinical Care Coordinator at
(203) 736-2601, Ext 322.

Exclusionary Criteria:
Sex offenders.

 Jail Diversion Program

Birmingham Group Health Services, Inc.'s Jail Diversion Program provides clinically appropriate evaluation and linkage with the Department of Mental Health and Addiction Services' (DMHAS) system and other community resources for court referred individuals.  Individuals referred to the program are pre-trial clients with symptoms of mental illness or current DMHAS clients for whom community care will provide an alternative to incarceration.

The Jail Diversion Program’s Forensic Team, consisting of a clinician and community support specialist, assess individuals at the Derby Superior Court.  The Team also works collaboratively with the Bail Commissioner to divert mentally ill offenders from incarceration to community care.

Participants in the Jail Diversion Program are provided with the following services: assessment, assistance in the development of conditional release plans, respite car and crisis management.
